66th Polonaise Ball and Scholarship Awards Dinner

General Information
Admission	$100.00 per person, by advance subscription. All proceeds benefit the scholarship/cultural fund.
Reservations	Please contact the committee by phone listed at the bottom.
	Remittance in full must accompany reservations. All reservations must be received by May 5, 2012.
Dress	Black tie optional.
Cash Bar	From 6 to 7 p.m., complimentary hors d'oeuvres will be available.
Dinner	Dinner will be served promptly at seven p.m.
Dancing	From 7 to 11 p.m. Dennis Ostopowicz Orchestra
Parking	Valet parking will be available.
